Bracknell Forest is a small unitary authority situated in the heart of Berkshire, which focuses on high quality, accessible, local services. Adult Social Care has effective partnerships with our local health providers and we deliver a range of services including Intermediate Care and Adult Social Care.

Here in Bracknell Forest Adult Social Care we have undertaken a major transformation of our services and there are currently vacancies for Senior Occupational Therapists on our Enhanced Intermediate Care Service (EICS) 

The EICS provides integrated health and social care input, supporting people in the community to maximise their independence, rather than relying on acute services. This will involve working as part of a large multi-disciplinary team. You will have supervisory responsibility, take part in triaging referrals and work as part of a shift pattern.

You will be working with health and social care professionals in the teams to ensure a high quality of work is delivered. This will play a significant part in helping us to respond to the needs of people in Bracknell.

You will be person centred and be committed to working as part of these integrated teams. Your caseload will be varied and complex. You will be required to work with people to promote their independence, health and wellbeing and in partnership with individuals (including unpaid carers) and those who support them. There is opportunity to take a lead in particular areas of development within the team.

You will undertake assessments, develop support plans with people and ensure the needs of people are effectively met through for example

 

  • Information, advice and signposting to community support
  • Reablement
  • Equipment
  • Minor and Major Adaptations
  • Long Term Support
  • Safeguarding

 

As a registered Occupational Therapist with the HCPC, you will be looking to broaden your horizons and make a real impact.
