Working within the Intermediate Care Team, our main aim is to help people stay in their own homes and supports individuals towards the end of their life.
We provide services to adults who require short term reablement, we support people in their own homes and in a residential setting. This varied role includes facilitating independence in daily tasks such as personal care, meal preparation, and rehabilitation programmes.
This team is a joint-funded service with health and social care professionals working closely together to provide a holistic approach to meet people’s needs within the personalisation agenda.
We are well known for our high standards of support, we will put into place a programme of training which will enable transition through the QCF in Health and Social Care and facilitate personal growth and development.
We are looking for people who are personable, have a good level of communication and practical problem solving skills. Ability to work under own initiative and liaise with other team members. Be able to motivate people and analyse changing situations.
Our service operates 365 days a year, and is a six week service. You will work within a set 3 week rolling rota which supports this approach, and this includes working 2 in 3 weekends. Hours across both the community and the residential service require some flexibility as the start and finish times for the shifts vary slightly.
Because this role involves visiting people in their own homes all applicants must have a valid driver’s licence and daily access to a vehicle.
